Post-polio syndrome: Doctors explain why weakness can return years after recovery

Polio is often spoken about as a disease that belongs to another generation. Vaccination has brought the number of cases down by more than 99 percent globally since 1988, and India has remained free of wild poliovirus transmission since 2011. But for people who survived polio in childhood, the illness may leave behind effects that emerge much later in life.

One such condition is post-polio syndrome (PPS), a non-contagious condition that can affect some survivors 15 to 40 years after their initial infection. It can cause new or worsening muscle weakness, fatigue and muscle or joint pain, even after a person has lived for decades without significant problems related to their original illness.

But does that mean vaccination isn't helpful?

Dr Nikhil Rajvanshi, Consultant - Pediatric Pulmonologist, Allergy and Sleep Specialist, Rainbow Children's Hospital, Delhi, says the importance of polio vaccination extends well beyond preventing an immediate episode of paralysis.

“Vaccination guards children not just against the immediate danger of paralysis, but potentially against a lifetime of disability that may only reveal itself decades later,” he says.

That makes completing the recommended immunisation schedule particularly important. The dramatic decline in polio cases can sometimes create the impression that the disease is no longer a concern, but Rajvanshi cautions against such complacency.

“Polio's current rarity is a direct result of sustained high vaccination coverage rather than a sign the threat has disappeared,” he says.

According to the World Health Organization, polio can still spread rapidly among unimmunised populations, and as long as poliovirus transmission continues anywhere, children in other countries remain at risk of importation and outbreaks. WHO says multiple doses of polio vaccine can provide long-lasting protection.

India was certified polio-free in 2014, although the country continues surveillance and vaccination efforts to maintain high population immunity.

“Missed doses, particularly in communities with existing immunity gaps, can leave children exposed once again,” Rajvanshi says.

He also stresses that parents should not ignore sudden weakness or floppy limbs in a child. Such symptoms require urgent medical evaluation because paralysis and weakness can be among the serious manifestations of poliovirus infection.

What is post-polio syndrome?

For people who contracted paralytic polio decades ago, the story can be different. Post-polio syndrome does not mean that the virus has returned.

“A common misconception is that post-polio syndrome represents a second infection or a resurgence of the polio virus itself. This is incorrect,” says Dr Mohit Anand, Senior Consultant - Neurology, CK Birla Hospital, Gurgaon. “The virus is not reactivating, nor is it contagious in this later stage,” he adds.

Instead, PPS is believed to be linked to changes in the nerve and muscle systems following the original infection. During the initial illness, poliovirus can damage motor neurons, the nerve cells responsible for controlling muscles. Surviving motor neurons can subsequently take over some of the muscle fibres that had previously been controlled by neurons that were lost.

Anand explains that these surviving neurons may then remain under increased workload for years. Over time, some can deteriorate, contributing to renewed weakness.

The CDC describes PPS as a slow, irreversible worsening of muscle weakness, often affecting muscle groups involved during the original infection. Muscle and joint pain are also common. Estimates suggest that around 25% to 40% of polio survivors may develop the syndrome, although the exact prevalence remains unclear.

The symptoms can be easy to dismiss

Unlike acute polio, which can cause sudden weakness or paralysis, post-polio syndrome generally develops gradually.

“Typical features include unaccustomed fatigue, slowly progressive muscle weakness, joint pain, and reduced tolerance for activities once managed with ease,” Anand says.

That gradual progression can make the condition particularly difficult to recognise. Someone who had polio as a child may initially attribute increasing tiredness, difficulty walking or reduced physical stamina to ageing, inactivity or another orthopaedic problem.

“These symptoms tend to develop gradually rather than suddenly, and can easily be mistaken for the ordinary effects of ageing,” Anand says.

Can anything be done?

There is currently no specific cure that reverses post-polio syndrome. Management focuses on preserving function, controlling symptoms and avoiding excessive strain on weakened muscles.

“Because post-polio syndrome is not widely discussed, many survivors dismiss early symptoms or attribute them to unrelated causes,” Anand says.

Recognising the condition early can allow patients and clinicians to adjust activity appropriately. Anand points to paced activity, physiotherapy and the appropriate use of mobility aids as part of sensible management.

Polio cases have fallen by more than 99% since the launch of the global eradication effort in 1988, with vaccines estimated to have prevented millions of cases of paralysis. Yet outbreaks can still occur when immunity levels fall, which is why maintaining high vaccination coverage remains central to eradication efforts.

For children today, completing their vaccination schedule is therefore about preventing an infection that may never cross their path. For those who survived polio decades ago, awareness of post-polio syndrome can help explain why an illness they thought they had left behind may still have consequences much later in life.
