Inspector General Anthony D’Esposito flagged the issue in a post on X on September 28, after an account called Expose H1B Fraud alleged that Rochester General Hospital had hired 82 resident doctors, of whom 80 were foreign workers hired on H-1B or J-1 visa.

He said that U.S. President Donald Trump’s executive order had put the visa abuse under microscope, adding that his office would would work with Vice President JD Vance’s White House Fraud Task Force if the system was being “gamed".

Scrutiny Over H-1B Visa

The episode comes as the Trump administration widens its scrutiny of the H-1B visa program beyond traditional technology-sector hiring. On September 18 this year, Trump extended the administration’s restrictions on certain H-1B workers for another year until September 21, 2027.

The proclamation restricts the entry of certain H-1B workers who are outside the United States unless their petitions are accompanied by a USD 100,000 payment, subject to specified exceptions.

The administration has also signalled the reshaping of the visa program. In an interview with American commentator and YouTuber Benny Johnson, D’Esposito warned that the H-1B visa program could look “very different" a year from now, siganlling a major crackdown on alleged foreign-labour fraud.

He said the administration was preparing a case that could lead to tougher enforcement.

“We are building a case. I think that the efforts and the focus that you see from the White House and from this administration is telling a story. We are laying out the case very vividly to the American people", D’Esposito warned.

“We have handcuffs ready", he added.

The H-1B visa program allows U.S. companies to hire foreign workers in specialty occupations, with the annual numerical cap limited to 65,000 new visas each fiscal year, alongside an additional 20,000 exemptions for individuals with a master’s degree or higher from a U.S. institution of higher education.
