Nagpur: In a rare and highly complex neurosurgical procedure, the Department of Neurosurgery at NewEra Hospital, Lakadganj, has successfully performed the en bloc removal of a giant 13-cm intradural intramedullary spinal cord tumour in a 20-year-old man.
The tumour extended from D9 to L3 and was diagnosed on histopathology as a Myxopapillary Ependymoma (CNS WHO Grade II).
The patient had been experiencing persistent lower back pain, pain in both legs, tingling, numbness, weakness and urinary urgency for several years. Detailed investigations revealed the extensive spinal cord tumour.

Six-Level Surgery for Giant Spinal Tumour
The challenging procedure involved a six-level laminotomy followed by laminoplasty, allowing surgeons to completely remove the tumour en bloc while preserving spinal stability and neurological function.

The surgery required meticulous microsurgical planning, advanced expertise and close coordination among the neurosurgical, neurophysician, anaesthesia and operation theatre teams.
According to the hospital, a review of published medical literature indicates that only a small number of cases involving en bloc excision of giant intradural spinal tumours have been reported worldwide.
The hospital stated that no published report was identified describing an en bloc excision exceeding 12 cm in the dorsolumbar region. The 13-cm tumour removed in this case therefore represents an exceptionally rare achievement in complex spinal tumour surgery.
Expert Team Performs Complex Surgery
The procedure was led by Dr Nilesh Agrawal, Director and Senior Neurosurgeon, along with Dr Vishal Pishe, Consultant Neurosurgeon.
The team also included Dr Aditya Atal, Neurosurgeon, and Dr Abhishek Wankar and Dr Ankit Panjwani, Neurophysicians.
The anaesthesia team comprising Dr Sahil Bansal, Dr Neha Nakade and Dr Aditya, along with the operation theatre nursing and technical staff, played an important role in the successful completion of the surgery.
Despite the size and complexity of the tumour, the surgical team achieved complete excision while preserving the patient’s neurological function.
Patient Recovers Well After Surgery
Following the procedure, the 20-year-old patient remained neurologically intact and ambulatory and showed an excellent postoperative recovery.
He was discharged in stable condition and will continue rehabilitation and follow-up care.
