Set during the winter of 1905, Man of Honor follows protagonist Enzo Favara in his early months as a trusted soldier for the Torrisi crime family. The expansion adds two new story chapters built around a delicate assignment: helping Ennio Salieri, the future Don from the original Mafia and Mafia: Definitive Edition, who has recently been released from prison and is looking to settle old scores in the Valle Dorata underworld.
The new missions explore Salieri's life before his eventual emigration to Lost Heaven, America, showing a younger, scrappier version of the mobster fighting to establish a smuggling operation. The expansion requires the Mafia: The Old Country base game and was first revealed at Summer Game Fest 2026.
